Employee Fuel Card Agreement Sample

Parties Involved:

Employer: XYZ Corporation
Address: 123 Business Rd., Cityville, State 12345

Employee: John Doe
Address: 456 Residential Ave., Cityville, State 12345

Fuel Card Details:

The Fuel Card issued under this agreement shall be used solely for authorized business-related fuel purchases, with card number ending in 7890, assigned to Employee John Doe.

Terms of Use:

The Employee agrees to use the Fuel Card exclusively for authorized business purposes, to not share or transfer the card to others, and to comply with all applicable company policies regarding fuel expenses.

Employee Responsibilities:

The Employee shall ensure that all fuel purchases are legitimate, retain receipts for reconciliation, and report any lost or stolen cards immediately to the employer.

Reimbursement & Payment:
